Personal Finance (Not Investing) • Form 5498 does not match 1099R

I did a direct rollover from a Roth 401k to a Roth IRA in 2023. I received a corrected 1099R showing the correct amount of withdrawal (after many months!) from the Roth 401k, but ETrade/Morgan Stanley has somehow managed to screw up the Form 5498. My 1099-R shows about $100,000 in distribution, coded H, and my Form 5498 shows a measly $24. They only included the amount of the cash that was rolled over, but not the fair market value of the securities that were rolled over in-kind.

What are my next steps? Should I call ETrade/Morgan Stanley and get them to correct the Form 5498 so that it matches with the 1099R? I don't want to get into trouble with the IRS and am not sure what to do next. I've already spent months on the phone chasing ETrade to even get the corrected 1099R and it's getting depressing.
